If you do not have a Healthcare Power of Attorney or Living Will in place, and are not able to communicate your healthcare decisions, the state of North Carolina requires healthcare providers to look to specific people in your life who are involved in your care to make these decisions on your behalf in good faith.

There is an alternative document, a MOST form, that would be an option for loved ones to complete on your behalf as it relates to your healthcare wishes.  To learn more about this document, click here to view our video about the MOST form.
